Custom Forms

The form builder lets you create custom forms for registrations, surveys, volunteer signups, prayer requests, and any other data collection need.

Creating a Form

  1. Navigate to Forms from the sidebar.
  2. Click Add Form.
  3. Enter a title and optional description.
  4. Add fields using the form builder (see field types below).
  5. Optionally set a capacity limit for maximum submissions.
  6. Click Save (forms start in Draft status).

Field Types

FaithAdmin supports nine field types in the form builder:

Each field can be marked as required and reordered by drag-and-drop.

Publishing a Form

  • Forms in Draft status are not visible to members.
  • Change the status to Published to make the form active and available.
  • Set a form back to Draft to stop accepting submissions.

Capacity Limits

Set a maximum number of submissions. Once the limit is reached, the form automatically stops accepting new responses. This is useful for event registrations with limited space.

Viewing Submissions

  1. Open the form.
  2. Click Submissions to see all responses.
  3. Each submission shows the respondent and their answers.
  4. Export submissions to CSV for analysis in a spreadsheet.

Linking Forms to Events

Forms can be linked to events for registration. See Event Registration.

Use the capacity feature for event registration forms so you do not have to manually close the form when spots fill up.
